TABLE PREFERENCE

Each 6 foot space is $15 for the evening.

Tables are to be provided by the vendors. However, we do have three extra tables that can be rented at an additional cost if needed. These tables will be available on a first come-first served basis. We will reach out to you to confirm your rental.

PAYMENT

Each 6 foot space is $15 for the evening. Your space is not guaranteed until payment has been received.

If you would like us to provide a table, it is an additional $10. Please confirm availability with us before paying for the table rental.

I consent to Photograph and Video to be used for promotional and social media purposes by The Grateful Self Care Company. Please read, date and provide your signature.

I hereby agree and give my permission for The Grateful Self Care Company and/or partners to record, film, photograph, audiotape or videotape myself (hereinafter collectively referred to as “Works”) and to display, publish or distribute these Works for the purpose of publishing, posting on the Grateful Self Care Company website and posting on social media sites. I hereby waive any right to approve the use of these Works now or in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of these Works. I understand that the Works may appear in electronic form on the internet or in other publications outside of the Grateful Self Care Company's control. I agree that I will not hold the Grateful Self Care Company responsible for any harm that may arise from such unauthorized reproduction.
